ICD Codes

Add ICD-10 and ICD-10-CA fields to your templates so Vero can surface the right codes from your note context.

Add an ICD-10 or ICD-10-CA field to a note template so Vero can suggest a code from the available encounter information. Choose the coding system used in your workflow and review every suggestion.

Add a code field to a template

  1. Open Templates and open the template you want to edit.

  2. Add the appropriate field below after Assessment or Diagnosis.

  3. Select Save in the template toolbar.

  4. Use the template for an encounter, generate the note, and review the proposed code against the documented assessment.

ICD-10 example

ICD-10 Code:
[Most clinically relevant ICD-10 code supported by this encounter]

ICD-10-CA example

ICD-10-CA Code:
[Most clinically relevant ICD-10-CA code supported by this encounter]

Review and correct a suggestion

Check that the diagnosis is supported by the encounter and that the code belongs to the coding system and current version you use. Edit or remove an incorrect suggestion before exporting the note.

Troubleshooting

The suggested code is wrong or missing

  • Confirm that the assessment and source information support the intended diagnosis.

  • Check that the template asks for the correct coding system.

  • Make the field instruction more specific, regenerate if needed, and review the result.

  • Correct the current note directly when required. Do not put one patient’s diagnosis code into a general reusable template as a fixed workaround.
