Confirm whether your jurisdiction or organization requires additional wording or documentation.
Store the completed form according to your organization's privacy and records-management requirements.

Access the Forms in Vero
Sign in to Vero.
Open Settings.
Select Forms & documents.
Find Patient Consent Form and select Download.
Select the patient's preferred language.
Signed-in users can also open Forms & documents directly.
