Templates define how your notes are structured. Set up a template once and Vero uses it to format future notes.
New to templates? Browse Discover first, find something close to your use case, copy it to your library, and customize from there.
Browsing templates
Open Templates from the left sidebar. The Templates page includes:
Personal - templates you created or copied.
Team - templates shared with your organization, when available.
Discover - templates shared by other clinicians that you can adopt.
Discover templates can include both text note templates and PDF form templates.
The three building blocks
Text - static content
Plain text appears word-for-word in every note. Use it for section headings, sign-offs, standard disclaimers, and anything that should not change.
Subjective:
Plan:
Dr. Sarah Chen, GP | Lakeside Medical
Keep required section headings as static text. If you need a History section, write History: in the template before the related fill-in field. A field such as [History] alone does not guarantee that the heading will appear.
Fill-in fields - [square brackets]
Square brackets tell Vero what to fill from your recording, written context, uploads, attached patient profile, and other available source material.
[Chief complaint]
[Medication name, dose, route, and frequency]
[Physical exam findings by system examined]
Be specific with your labels. [Medication name, dose, route, and frequency] gives better results than [Medications].
Rules - (parentheses)
Instructions in parentheses guide Vero's formatting and behavior. They do not appear in the final note.
(Use DD/MM/YYYY for all dates)
(Only include if explicitly mentioned)
(List vitals in one line)
You can put rules beside a specific field or at the end of the template when they apply to the whole note.
Example template
Subjective:
(hyphenated list)
- [Brief statement of chief complaint or reason for visit]
- [Relevant associated history in chronological order]
Objective:
(hyphenated list)
- [Vital signs with units in one line]
- [Physical exam findings by system examined] (Only include systems examined.)
Assessment:
(hyphenated list)
- [Diagnosis and reasoning] (Only include active issues being managed today.)
Plan:
(hyphenated list)
- [Investigations planned or ordered]
- [Treatment plan]
- [Counselling discussion]
- [Follow-up plan]
- [Return precautions]
Quick principles
Be specific about formatting.
(Use hyphenated bullets)is clearer than(Use bullets).Split large fields.
[Clinical impression]and[Plan]are usually better than[Full assessment and plan].Prevent assumptions. Add
(Only include if explicitly mentioned)when you want Vero to omit unsupported content.Keep variable details in square brackets. Patient-specific facts, medications, diagnoses, follow-up timing, and pronouns should usually be fields, not fixed text.
Iterate. Use the template in a real session, review the output, and refine only what did not work.
Creating a template
Select Create template, then choose Text Template or PDF Template. Text Template offers the first four methods below; PDF Template opens the form-upload flow:
Method | When to use it |
Blank Slate | You know the exact structure you want. |
AI-Generated Template | Describe the type of note you need and Vero creates a starting template. |
Use an Existing Note | Paste a past note and Vero turns its structure into a reusable template. |
Import a Template | Paste a template from another source and Vero adapts it to Vero's format. |
PDF Template | Upload a fillable PDF form for Vero to complete from encounter context. |
Editing templates
Open Templates, find the template in Personal, open it, then edit the template content. Common tweaks include adjusting date formats, adding clearer fields, adding formatting rules, changing headings, or reordering sections.
Save text template changes: click Save in the template toolbar. Closing the editor does not save the change. If you try to leave with unsaved changes, Vero asks whether to save, discard, or stay.
Templates are private by default. Team and public sharing depend on your account and organization settings. Public templates appear in Discover. Review any template before sharing it.
For sharing controls and requirements, see Sharing Templates.
Edit with Chat
You can ask Chat to help edit the open template. In the template editor, review the changes and select Save. In an encounter, explicitly asking to change the current template for future notes saves a successful supported edit as a new version. Review the saved result in Chat; your existing note is not automatically regenerated.
Improve a template with Learn
After generating a note, edit its structure or style, then click Learn in the note toolbar. When Vero finds a reusable change, it saves an updated template version. Edits for one patient should stay in the encounter rather than becoming fixed template content.
For a template owned by someone else, Vero asks before creating or updating your personal Learn copy. Select that copy for future notes. A template you own is updated directly, even if you have shared it.
Review or restore a version
Open Templates and open the template you want to restore.
In the template toolbar, select Version and choose the earlier version number you want.
Review the content, then click Save to make it active for future notes. Selecting a version on its own only previews it.
Restoring a template does not rewrite notes you have already created. Keep any unsaved template edits before switching versions.
Learn-created versions are marked Learned. For the automatic template changeover and how Learn differs from before, see Vero Learn. If you need help adapting your template, ask Fin in the in-app support chat for steps or a suggested instruction you can review and apply.
Frequently asked questions
Is Use an Existing Note the same as uploading a document?
Is Use an Existing Note the same as uploading a document?
No. Use an Existing Note takes text pasted into the template-creation flow and uses its structure to create a reusable text template. Import a Template also uses pasted text. Uploading a Word document to an encounter adds source material for a note; it does not create the reusable template through either of these methods. PDF Template is the separate form-upload flow.
What if template creation fails?
What if template creation fails?
Keep a copy of any unsaved text. Once it is safe to retry, try the same creation method once. If it fails again, stop retrying and ask a teammate in your existing support conversation to investigate. Include the exact error, creation method, and device/browser. You do not need to paste the clinical note into the support chat. See Errors & Template Issues.
Do I need to save a template change made in Chat?
Do I need to save a template change made in Chat?
When you ask Chat in an encounter to update the current template for future notes, a successful supported edit is saved as a new version. Review the saved version shown in Chat. For a Vero-provided template, the change is saved to your personal version.
Inside the template editor, Chat applies changes to the unsaved editor. Review them and select Save. Updating a template does not automatically rewrite the note you already created; request regeneration separately if needed.
Can I create a separate template from encounter Chat?
Can I create a separate template from encounter Chat?
Encounter Chat cannot create a separate template while you have a note open. Open Templates → Create template, choose Text Template, then use Use an Existing Note to paste a note or AI-Generated Template to describe the new template. This keeps the current template unchanged.
