What Patient Profiles do
Patient profiles keep a patient's identifying, contact, and clinical information together across visits. One profile can be attached to multiple encounters, making it easier to review prior history, create follow-up encounters, and use relevant patient context in Vero.
Attaching a patient is optional, but patient-aware features only use information that is stored in Vero and linked to that patient.
Create a patient profile
From the patients page
Select Patients in the sidebar.
Select New Patient.
Enter the patient's name. The name is required; the remaining fields are optional.
Add any available demographic, contact, address, or clinical information.
Select Create Patient.
From an encounter
Open an encounter and select the patient control near the encounter title.
Search for an existing patient and select the correct profile.
To create a new patient, type the patient's name and select [patient name] (add patient).
When an encounter still has its default title, attaching a patient renames it to the patient's name. A title you entered manually is preserved.
Import multiple patients
To create or update multiple profiles at once, select Import Patients on the Patients page and upload a CSV file. See Importing Patients for the required columns and review process.
Manage a patient profile
Open a patient from the Patients page to view and update their profile. The patient name is the only required field. Age is calculated from the date of birth when one is entered.
Profile information
Demographics: name, date of birth, sex, pronouns, medical record number, and other identifiers.
Contact and address: email, phone, country, address lines, city, province or state, and postal or ZIP code.
Clinical information: medical history, medications, allergies, family history, social history, and other available clinical sections.
Custom fields: additional sections created for your workflow.
Patient page
The patient page brings the patient's work together in one place. From here you can:
Edit profile and clinical information.
Create a New Encounter with the patient already attached.
Review upcoming and recent encounters and open their notes.
Create and manage patient tasks.
Review patient documents and import information into the profile.
Import information from patient documents
From a patient page, select Import to upload PDF, Word, image, or audio files. Vero extracts possible profile updates and lets you approve each field before saving. See Importing Information Into a Patient Profile.
Always verify generated snapshots and extracted information against the source record before using them for clinical care.
Find and review patients
The Patients page supports repeated list-management workflows:
Search by patient information.
Filter by sex, age range, or last visit.
Sort supported columns such as name, visit activity, and last updated date.
Open the analytics view to review patient-level charts using the active filters.
Select New Visit in a patient row to start an attached encounter directly.
To prepare several planned encounters from a schedule, use Import Schedule.
Link patients and start follow-up encounters
Start a new visit for an existing patient
Open Patients and select the existing profile.
Select New Encounter within the patient profile.
Confirm that the patient is attached, then record or add the visit information.
Use the same patient profile for daily or later follow-up visits. Each visit can have its own encounter; you do not need a new patient profile each time. The New Encounter button in the main sidebar starts without a patient attached, even when you have a patient or earlier note open.
Attach a patient to an existing encounter
Open the encounter and select the patient control near its title.
Search for and select the existing patient profile.
Confirm that the selected patient appears on the encounter before continuing.
If you entered an encounter title manually, selecting a patient preserves that title. Check the patient control to confirm the link rather than relying only on the title.
Use the attached patient's information
After attaching a patient, open their panel from the patient control in the encounter header. The panel gives you access to profile details, linked history, and patient-aware actions without leaving the encounter.
Patient Snapshot
When prior linked history is available, Patient Snapshot provides a concise longitudinal recap based on information already stored in the patient profile and previous encounters. On the encounter Context page, select Add to Context to include the reviewed snapshot in the current encounter context.
Extract from context or note
Vero can suggest patient-profile updates from the current encounter:
Select Extract from Context while viewing the encounter Context.
Select Extract from Note while viewing a generated note.
Review, edit, and accept the suggestions you want to save. Vero may suggest adding information or replacing an existing value, but nothing is saved until you accept it.

Use patients in chat and evidence
In Vero Chat or Vero Evidence, use the + menu and select Patient to add patient context. Chat can also support patient and panel workflows using the patient information and encounters available in Vero.
Vero cannot see information that has not been entered, uploaded, or linked in Vero. Confirm that the relevant patients and encounters are present before relying on a panel or roster review.
Export a patient record
Open the patient profile and select Export near the top of the page to create a combined PDF.
Choose the date range for the encounters you want to include.
Select Clinical information, Notes, Documents, or Select all. Options without available content are disabled.
Select Export PDF and wait for the download.
Review the PDF before storing or sharing it.
The date range filters encounters for the Notes and Documents options. Clinical information comes from the current patient profile.
Delete a patient
Open Patients.
Select the trash icon in the patient's row.
Review the confirmation message and select Delete.
Deleting a patient permanently removes the patient and all associated encounters, notes, and tasks. This action cannot be undone.
Troubleshooting
The selected patient does not stay attached
Selecting an existing profile should attach it to the encounter. If that selection fails or does not persist, keep the encounter and ask Support to investigate. Do not create duplicate patient profiles as a workaround. Share the affected encounter link, approximate time, device, and what you selected; avoid unnecessary patient details in the support message.
If an encounter appears to be missing
Search the encounter list, check Archived, and confirm that the encounter is attached to the expected patient before assuming it was deleted. Follow Finding a Missing Encounter for the full recovery workflow.
